For the PPE Form document, you must bring all 5 pages to your physician for the Physical Evaluation. Parents/Guardians and student-athletes must fully complete pages 2-5 and provide to your physician. The physician should return only the first page of the document, The Medical Eligibility Form, back. This Medical Eligibility Form is the form which needs to be turned into the Roselle Catholic Health Office.

These sheets are required of all student-athletes prior to participation in any sport or activity at Roselle Catholic. Each page needs to be signed and dated by the parent and the student. The sign-off sheets have been compiled into one document to make it easier to print all at once.
